Torsion of a pedunculated subserous leiomyoma in pregnant woman is rare condition that requires prompt diagnosis and urgent surgical intervention. In this report, we present case torsion primigravid aged 33 years her 30th week gestation. A presented to our clinic gestation with pain the right lower quadrant had developed last 24 hours. Our was leiomyoma. The patient received tocolytic nifedipine indomethacin preoperatively. Given volume uterus as precaution potential complications, transverse (pfannenstiel) incision preferred. laparotomic myomectomy successfully performed pregnancy continued uneventfully. Even though very condition, intervention life saving provides more favorable maternal fetal outcomes. approach should be tailored characteristics myoma an expert team surgeons anesthesiologists essential order reduce risk complications.
